So there was an issue with those fields being taken away as the date and time on the platform got reset, very odd but I have listed below the steps to get this back. If this is not clear I suggest calling in and one of our analysts can also assist
1. Sign into your fulfillment account 2.In the top right on the blue banner of your page you should see a tile icon with a bunch of different smaller squares, click on that and select launchpad. 3. From there, you should see listed up top the following options- Home, Profile, and Settings---if you do not see settings you should do a hard refresh (control, f5) and it will be listed there. 4. Go ahead and click on settings and you should see the time settings, these will have to be reset. 5. In order to reset this, all you need to do is change the locale time to one option and then bring it back to English (US 12 hour), it is strange but essentially something just needs to be changed and then changed back to allow you to select "apply changes" at the bottom as it will be greyed out until a change is made.
Once you do that, the date information should be back on the form, I know this is a bit confusing and a lot of steps so once again if this was not clear enough I do apologize and recommend calling into our line 1-888-739-3232 and they will be able to further assist.
